You have an overseas trip postponed five months due to their neglect

It all started out when I needed a new doctor because I was too old for my pediatrician. I went to my moms doctor (she had only been a patient of hers for a month maybe) and it all went downhill from there. I needed paperwork filled out for a nonprofit overseas trip. I warned the office that it would take a while and they said OK. I needed blood-work done and she never called to check if it was done so I eventually had to go get it myself. I moved after a month. All she had to do was fill out the paperwork from my last visit and mail it to me, but when she did almost everything was blank. I went ahead and got everything checked with the nonprofit so I would know what exactly needed to be fixed: she left the immunization records blank, the GYN section, signature spots..etc. I called to have it fixed but she said she didn't have time for it anymore!

When I talked to the hospital she worked out of they made her schedule an appointment with me to fix it. When I went in she was so rude and unprofessional and she had to give me a pelvic and rectal exam which made it way worse. She spent half the time complaining about how she didn't want to do it and I kept trying to keep her from freaking out because I just wanted to get it over with. When I left I was no longer covered on insurance because I had just graduated college so they wanted to charge me $400 for her to finish something that she should have done already! I ended up paying $200 after complaining to the office manager.

When I mailed in my paperwork to the organization they lost it (I used a tracking device that said they received it but they said it was lost in their building). I called my doctor to see if they could send me a fax of it (I should have kept a copy myself the first time but the only place I could find to copy it was very expensive and I was short on time) so I could keep it on record and to see if they could fax a copy to my office (it would probably take them 5 minutes if they were organized) and they said they couldn't find my files! I called back 2 days later and they said they didn't have time for it. I'm going to call again tomorrow and after that, I'm not sure what I will do.
